Sport Injury Health Insurance

This plan provides essential medical and health insurance for amateur athletes in Canada for only $100 per month for international players and $25 per month for Canadian players. This plan is underwritten by AIG Commercial Insurance Company of Canada.

Overall coverage limit: $1 million per person per policy.

Basic Medical Benefits

Hospital benefits; up to overall coverage limit
Medical, surgical and diagnostic benefits; up to overall coverage limit (except limit of $10,000 on private duty nursing)
Ambulance services (including mountain and sea rescue); up to $10,000.00. Transportation expense for a licensed taxi up to $100.00 for any one sickness or injury per insured person
Emergency air ambulance transportation when approved in advance by the insurer; up to a limit of $100,000

Extended Medical Benefits

Prescription drugs when prescribed for an emergency; up to the overall maximum but not to exceed a 30-day supply.
Specialists (physiotherapist, chiropractor, chiropodist, osteopath or podiatrist) when deemed essential by the attending physician; to a maximum of $600 per policy per practitioner
Accidental dental treatment; up to $2,000
Emergency non-accidental dental treatment; up to $600
Extraction of impacted wisdom teeth; up to $100 per tooth

Transportation, Death & Other Benefits

Early return to country of origin when deemed necessary by the attending physician; limit of $100,000
Repatriation of remains; limit of $10,000
Burial or cremation at place of death; limit of $5,000
Accidental death & dismemberment insurance; limit of $25,000
Air Flight Common Carrier Accident insurance; limit of $100,000
